医療現場のランチャート管理を効率化するRパッケージ「runcharter」の紹介

So, after way too many false starts (and honestly, more procrastination than I’d care to admit), I finally built my first public R package: *runcharter*. It’s a niche little thing—like, *counting interested people on one hand* niche—but if you’re in healthcare QI drowning in runcharts, you might get it.

The problem? Updating medians for hundreds of charts is a spreadsheet nightmare. I’ve hacked at this in SQL, BI tools, even blogged about it years ago. This package automates the pain: feed it a dataframe, and it hunts for runs of points above/below the median, recalculates medians, and spits out faceted plots (my record? A glorious 68-panel monstrosity that made me weirdly emotional).

Originally, I only cared about one-sided runs (e.g., fewer patient falls), but 66% of early testers begged for two-sided checks, so… fine, it’s in there. The code’s still rough—namespace quirks kicked my butt, and *testthat* remains a mystery—but it *works*.

Shoutout to *qicharts2* and *runchart* (way fancier than mine) if you need alternatives. But hey, if you’re as obsessed with runcharts as I am, give it a whirl. Just don’t judge the internals too hard.

あのさ、このruncharterパッケージを日本で広めようとしたら、まず壁になりそうなのが「医療現場のカルチャー違い」かな。日本の病院ってExcel信仰がすごくて、「Rなんて触ったことない」って人が多いんだよね。私が「自動で中央線更新できますよ」って言っても、「え？今まで手動でやってたけど何か問題？」って顔されそう...。

それに、ランチャートの概念自体が浸透してないかも。「管理図」って言った方が伝わるのかな？ でも英語のquality improvement用語をそのまま訳すと、なんか堅苦しくなっちゃうし。この辺のニュアンスをどう説明するかが悩ましい。

技術的なハードルもあって、例えば「テスト駆動開発？それよりとりあえず動くものが欲しい」みたいな空気があったり。testthatが分からなくてもパッケージ作っちゃう私みたいな人間が増えるのかも（笑）。あと、クレームつけられた時のために英語のドキュメントを日本語化しないと...と思いつつ、専門用語の訳語が定まってなくて悶々としてたり。

でもまあ、68枚のグラフが一瞬で描けるのを見せたら、「お前...すごいな」って言われるかも、という淡い期待は捨ててないよ。Excel地獄から救える人が一人でもいれば本望だよね。
